Knee arthrodesis – ultima ratio for the treatment of the infected knee
نویسنده
چکیده
The irretrievable destruction of the knee due to trauma, tumor or infection is the indication for knee arthrodesis. The main reason for knee arthrodesis in terms of infection ist the infected total knee arthroplasty. Central problem is the definition of the term "irretrievable". It is based on the subjective opinion of the attending physician and depends on his expert knowledge of this specific entity. The preservation of a functioning extremity is the main goal. This article shows the typical indications and contraindications for knee arthrodesis following septic knee diseases. In addition it gives insight into the biomechanical and technical considerations to be kept in mind. Finally the postoperative care and outcome of different techniques are analysed.

Distraction osteogenesis as a salvage method in infected knee megaprostheses.
Infection of total knee replacement represents a severe complication. Especially in cases of infected megaprostheses, treatment options are limited and even amputation may become unavoidable. We present two cases of infected knee hinged megaprostheses. Both were treated by prosthesis removal and debridement of all surrounding infected bone and soft tissue, followed by distraction osteogenesis f...
متن کاملKnee Fusion or Above-The-Knee Amputation after Failed Two-Stage Reimplantation Total Knee Arthroplasty
Prosthetic joint infection (PJI) is a serious complication of total knee arthroplasty (TKA). Control of infection after a failed two-stage TKA is not always possible, and the resolution of infection may require an above-knee amputation (AKA) or a the-knee (KF). The purpose of this review is to determine which treatment method (AKA or KF) yields better function and ambulatory status for patients...
متن کاملArthrodesis Using Pedicled Fibular Flap After Failed Infected Knee Arthroplasty
OBJECTIVE Severe bone loss associated with failed revision total knee arthroplasty is a challenging scenario. The pedicled fibular flap is a method to obtain vascularized bone for use in knee arthrodesis after failure of a total knee arthroplasty, with substantial loss of bone. METHODS We report 2 successful knee arthrodeses using this method in patients with infected, failed multiply revised...
متن کاملArthrodesis of the knee after an infected arthroplasty using the Ilizarov method.
We treated 15 patients by arthrodesis of the knee after removal of an infected total knee arthroplasty, using an Ilizarov ring fixator. Eight had a failed arthrodesis by another technique. The mean age of the patients was 75 years, the mean duration of retention of the frame was 28 weeks, the mean treatment time 51 weeks, and the mean follow-up 52 months. All but one knee fused at the first att...
